Because the totto lands are a great place for civilians to live otherwise. Theres a stable and effective government, the islands are rich, the people are cared for by the ministers, it is safe from invaders and threats, and most if not all of the executives of the Big Mom's pirates arent actually bad people. The only downsides are having to give ur lifespan, which isnt even much since they dont ask for years, and Big Mom's hunger pangs which dont seem to happen too often. Other than this current hunger pang, we've only seen it happen once unprovoked.

I think at some point during this arc, someone who knows Big Mom very well like Jimbei or Big Mom herself said that no one can leave her territory alive once they're inside, at least not unless they're allies I presume. Basically it's easy to get in, but hard to get out. I don't know why anybody would want to move there but I can bet no ordinary person is leaving even if they want to.
